The State of New Jersey now requires that all children 11
years old and up beginning 6th grade must have the Tdap
and Meningococcal vaccine. Please call the office to schedule
your child's annual physical to make sure they are receiving all
of the required vaccines annually.


In addition, we are offering the following:
Human Papilloma Virus (Gardasil)
Hepatitis A (manufacturer limited supply until 2008)
Flu Vaccine

All Influenza Vaccine will be administered during scheduled physicals or
flu-clinic only, on a first-come, first-serve basis.

Newborn in hospital: Hepatitis B
5 weeks old: Hepatitis B           
2 months old: Pentacel #1(Polio, DTaP, Hib), Pneumococcal #1
4 months old: Pentacel #2(Polio, DTaP, Hib), Pneumococcal #2
6 months old: Pentacel #3(Polio, DTaP, Hib), Pneumococcal #3
9 months old: Hepatitis B
12 months old: Varicella, Pneumococcal #4
15 months old: DTaP #4, Hib #4
18 months old: MMR

5 years old: DTaP #5, Polio #4, MMR #2

5 years and up: Varicella #2

11 years old: Tdap, Meningococcal

12 year old and up, girls: HPV (3 shot series over 6 months)

Annual influenza vaccine by request any child 6 months old and up
